Premier Alejandro Goya, a dictator who wants to open his country for democratic elections, is treated at Westside Medical Hospital, where Captain Awesome and Ellie work. The team is assigned to protect Goya during his stay, while Chuck has to gain some inside medical information about Goya by using Devon.

UCLA med school lab circa 2000: Ellie and Awesome are in a broom closet making out and getting naked instead of attending their class.
Back in the present day, Ellie and Awesome are moving into their new apartment. Chuck drops in after getting back from a mission and fixes their TV. Chuck flashes on a news report about the Costa Gravan leader, Premier Alejandro Goya, and learns that this dictator has been a person of interest to the U.S. government for quite some time. Presently, Premier Goya has been hospitalized. Chuck must report to Castle, while Awesome gets called to the ER.
Beckman explains that despite earlier attempts by the U.S. to assassinate the Costa Gravan leader, Premiere Goya is now an important ally -- and one who must be guarded. Casey bristles at the thought of protecting Goya -- the communist despot he tried to kill on several different attempts. Beckman explains clearly: they're to guard Premier Goya at all costs and Captain Awesome must do everything he can to save his life.
Awesome is an awesome cardiologist and saves Premier Goya's life. Chuck confronts Awesome in the courtyard and the two deduce that the Premier was poisoned. Awesome volunteers to help the CIA because he needs some real adrenaline, but Chuck says that Ellie would kill him if he ever put her awesome husband in danger. Later, General Beckman tells the team that she is concerned the assassin is on the loose and they must infiltrate a gala at the Costa Gravan consulate to protect the dictator.
Casey recuses himself from the mission because he is a wanted man on Costa Gravan soil. He was known as the "Angel de la Muerte" in the '80s. Suddenly, Premier Goya's men are outside, but they're not there for Casey. Chuck realizes that the men are there for Awesome, who is busy working out on his stationary bike. Awesome opens the door and Premier Goya greets him with open arms and invites the awesome doctor and Ellie to the gala at the consulate. Chuck is able to also get invited by "stumbling" into the conversation.
At the gala, Sarah assures Chuck that she'll be able to protect Premier Goya, but Chuck is more nervous about Awesome and Ellie. Outside in the surveillance van, Casey keeps a wary eye and radios Chuck to watch for assassins. Awesome asks Chuck if he has a mission, but Chuck tells him to keep Ellie out of danger. But it might be too late: Premier Goya is talking to Ellie and has taken quite a liking to her. The dictator wants Ellie to be by his side the whole night, and maybe even some time after the party.
Sarah pulls Ellie to safety away from the dictator, but then gets an earful of relationship advice from Ellie. Ellie thinks Sarah might still have feelings for Chuck and Sarah does her best to be vague about their relationship.
Premier Goya gives his speech and invites Ellie to dance with him, making them both open targets for the assassin. Casey uploads a photo of a possible assassin to Chuck's phone. Chuck and Sarah see the suspect across the dance floor and Chuck flashes -- he needs to get some serious dance skills. While Chuck and Sarah do a forbidden dance, Chuck manages to knock the suspect out, who happens to be an innocent protestor. Sarah and Chuck are kicked out by the head of security and Chuck flashes on a revolutionary guard, who turns out to be the real assassin, but Chuck and Sarah are locked out and have to head back to Castle.
Casey knows what he has to do: he arrives at the consulate dressed as a revolutionary guard. Casey spots the assassin, who has a syringe in hand. Casey pulls out his gun, but Awesome, not recognizing him, tackles him to the ground and scares the real assassin away -- Awesome now looks like the hero. The head of security recognizes Casey right away as the Angel de la Muerte and apprehends him.
General Beckman orders Chuck and Sarah to stand down: the government can't risk their relations with Costa Gravas and Casey will have to handle his escape on his own. In a holding cell, Premier Goya talks to Casey, the Angel of Death, who is bound to a chair. Casey tries to tell Premier Goya that he was there to protect him, but the dictator doesn't believe him: in the past Casey has shot him in the spleen and blown up his dog.
Premier Goya won't listen to Casey and gags him just in time for the real assassin to inject poison into the dictator's cigar. When the assassin goes to light it for Premier Goya, Casey, unable to speak, kicks the cigar out of Premier Goya's mouth. But Goya picks it up and lights it, inhaling the sweet smoke of the Costa Gravas cigar before passing out. Casey is now left in the holding cell alone and bound with the assassin.
Ellie thanks Awesome for a wonderful night and just as the two are about to spend some quality alone time, Awesome gets a call from the consulate and has to leave to attend to Premier Goya. Awesome tells Chuck that he can get them into the consulate and get Casey back. Chuck agrees and brings Awesome to Castle; something Sarah is not too pleased about! However, once Sarah understands that Awesome can get them into the consulate, she has a plan: they will dress undercover as Awesome's medical team to infiltrate the consulate and save Casey.
Meanwhile, the assassin visits Casey in the holding cell and informs Casey that he works for The Ring: Costa Gravas must remain an enemy to the United States. Just as the assassin is about to inject Casey with the poison, Casey head-butts him. In Premier Goya's room, Sarah takes out the two guards so she and Chuck can find Casey and rescue him; all Awesome has to do is tend to Premier Goya. When Awesome sees Sarah's fighting skills in action, he is more than impressed. However, the fight between Casey and the assassin is not going too well when the poison-filled syringe gets stuck in Casey's leg.
Casey breaks free of his chair and clubs the assassin with it, knocking him out. Unfortunately for Casey, a guard hears the scuffle and shoots him in the leg. However, Sarah and Chuck get there just in time and there's only one last thing to do: get Awesome. But when the three get to Premier Goya's room, the head of security is there with backup. Casey, who is shot, passes out. In the holding cell, the assassin has disappeared. Even though Chuck tries to prove their innocence, the head of security wants Chuck (who is still dressed as a doctor) to operate on Casey. This is the only way he can prove he is not an assassin.
Casey is not allowed anesthesia and Chuck has to operate on Casey's bullet wound. Awesome tries to tell Chuck that the bullet might have grazed the femoral artery, but the head of security wants Chuck to do this all by himself. Once Sarah and Awesome calm Chuck's nerves he flashes on everything he needs to know about medicine and surgery, becoming Dr. Bartowski and extracting the bullet from Casey's leg.
Premier Goya starts to flat-line and Awesome says the dictator needs a blood transfusion. Lucky for Premier Goya that Casey not only knows Premier Goya's blood type (AB negative), but he also has the same blood type (which Chuck finds out by taking a look at Casey's dog tags). Casey protests and will not give up his blood to Premier Goya, a man he has been trying to kill for years. Chuck makes the executive decision for Casey and knocks him out with anesthesia, taking the blood by force.
Once awake, Casey is congratulated by General Beckman for saving Premier Goya's life. Since Premier Goya could not thank Casey personally, he sent him a box of Costa Gravas cigars and calls Casey the Angel of Life. Later, at the Orange Orange, Awesome tells Chuck that being a doctor is all the excitement he needs.
Chuck and Sarah simplify their relationship cover and decide to be friends. At the hospital, Awesome is taken by the assassin. Later that night Sarah goes to Chuck's apartment to tell him about Awesome. Chuck is shaken by the news and what's worse is Ellie is looking for her husband. But Awesome is officially missing.
